Complex Family Planning Fellowship Overview

In 1970, Hawaiʻi became the first state in the Union to allow abortion at the request of a woman. Since that time, the UH faculty has been a significant provider of abortion care. In 2006, we furthered our commitment to providing evidence-based family planning and abortion services through the successful implementation of our Kenneth J. Ryan Residency Training Program. In 2012 we were proud to become the 25th Family Planning Fellowship site.

The UH Fellowship in Complex Family Planning aims to develop exceptional CFP subspecialists who also maintain and grow in their expertise as ob-gyn generalists. The two-year, ACGME-accredited Fellowship program follows the educational, research, policy, and clinical training requirements outlined by the Society of Family Planning. Our Fellows

  • Provide clinical care in both hospital-based and community-based settings on multiple islands
  • Develop and complete an impactful research project that aligns with their interest and passion
  • Participate in multiple policy and advocacy projects locally and nationally
  • Engage in professional development opportunities locally and with their national cohort of Fellows
  • Manage complex patients in collaboration with our generalist and MFM colleagues and provide care in Hawaii’s only Level 1 trauma center
  • Care for patients who reflect the diverse cultural makeup of the state and come from both Hawaii and the broader Pacific region for care.

Graduates of the Fellowship are experts in complex family planning clinical care, family planning clinical and translational research, family planning public health and public policy, and family planning education. Fellowship graduates are highly sought after in academic departments of Ob-Gyn throughout the country.

Fellows can expect to have hands-on training in:
  • All methods of pregnancy termination
  • All diagnostic methods to confirm uterine extra-uterine pregnancy
  • Anesthesia and pain control for in-office gynecologic procedures
  • Ultrasonography related to contraception and abortion
  • Management of complications
  • All methods of contraception currently available and under investigation
  • Generalist call (additional hourly pay)*
  • Permanent contraception
  • Deep implant removal- UH is a Contraceptive Implant Center of Experience
  • Opportunity for vasectomy training
  • Working in high-volume freestanding clinicsWorking in rural/low-resource settings

* 24 hour minimum requirement per month; call is at The Queenʻs Medical Center and includes both L&D and gynecology.

CFP fellows teach medical students and residents routinely, both on CFP rotation and when doing generalist call. Fellows are also encouraged to work with residents or medical students who are interested in CFP research. The University of Hawaii Ob-Gyn residency program has 24 categorical positions. The University of Hawaii residency program is a Ryan Residency Program, and residents do a CFP rotation in their intern year.

Research is central to the focus and structure of the University of Hawaiʻi CFP division and Fellowship. The entire division, including all faculty and fellows, meet weekly to brainstorm research ideas, identify funding opportunities, discuss research progress and troubleshoot barriers. The Division also has one dedicated PhD researcher and one dedicated full time research coordinator. Biostatistics help is available when needed. Fellows also receive funding each year to attend research courses, such as the SASCOG Progress Course, and have funding to attend professional conferences, both in Hawaiʻi and on the continent. Fellows are encouraged to identify their own research questions, and the breadth of methodological expertise among the faculty ensures fellows are supported in producing high quality research, no matter the design. The UH CFP Division is hands-on and engaged with the fellows, providing support for study design, IRB approval, study implementation, analysis and dissemination. Fellows typically complete one primary and one secondary project during their fellowship.

As the first state to legalize abortion at the request of the patient in 1970, Hawaiʻi has a proud history of protecting abortion rights. UH faculty and fellows have been integral in legislatively reaffirming the right to abortion, and proactively codifying protections in the years following the Dobbs decision. UH faculty and fellows work with community organizations, reproductive health advocates and state legislators prior to the legislative session to craft bills and meet with stakeholders. During the legislative session, faculty and fellows provide written and oral testimony and assist the ACOG junior fellows in planning the ACOG Hawaii Section lobby day. Our fellows have participated in multiple media opportunities as well.

We have formalized advocacy training into a curriculum that emphasizes not only legislative advocacy and media training, but institutional advocacy as well.

Fellows are encouraged to pursue elective rotations that support their educational goals. Past fellows have done rotations at DuPont Clinic for all-trimester procedure training and with the Population Council to pursue additional research experience. Fellows are encouraged to explore elective opportunities and will be supported as much as possible in completing electives that match their interests and goals.

The UH Division of Family Planning is proud to be a participating Ryan Residency Program. Residents complete an opt-out 8 week family planning rotation and complete a thorough didactic and clinical curriculum, with in-depth evaluation of knowledge and competency throughout. Prospective residents can learn more about the UH Department of Ob-Gyn Residency at https://hawaiiresidency.org/

  • Curriculum: Residency program graduates are expected to meet competencies in patient education and counseling regarding contraception and abortion, early pregnancy loss and IUFD management, and medical and surgical pregnancy termination. Residents will participate in didactic sessions regarding epidemiology, public policy, research and clinical care in family planning.
  • Research: The Family Planning division is actively involved in resident research mentorship. Resident research in family planning has included topics such as post-placental IUD placement in adolescents, emergency contraception pharmacist knowledge and pharmacy availability and LARC uptake among Native Hawaiian women.

Medical students work with the Family Planning division during their 3rd year Ob-Gyn clerkship. Students participate in a hands-on lesson on contraception, IUD placement, and uterine evacuation, as well as a discussion about professionalism and challenging patients. Medical students will participate in clinical cases involving family planning, and can arrange to participate in office-based family planning services during their rotation. Visiting Medical Students: Space is available for medical students to rotate with the Department of Ob-Gyn and to participate in a family planning rotation. Students will receive a tailored reading and didactic curriculum and will participate in office- and hospital-based family planning cases.
